For Type 2 Diabetes Mellitus:
Conventional (Immediate-Release) Formulation:
Start with 500 mg twice or three times daily, or 850 mg once or twice daily.
Increase gradually to a maximum of 2000 mg/day, adjusting every 1 week.
Maximum dose: 2550 mg/day.
Modified-Release (Extended-Release) Formulation (e.g., Informet XR 500):
Start with 500 mg once daily, preferably at night.
Increase by 500 mg weekly as needed.
Max dose: 2000 mg once daily at night.
If blood sugar is not well-controlled, split dose to 1000 mg twice daily.
Do not exceed 2000 mg/day.
For Polycystic Ovary Syndrome (PCOS):
Week 1: 500 mg once daily (morning)
Week 2: 500 mg twice daily
Then increase to 1.5–1.7 g/day in 2–3 divided doses based on tolerance.
Special Populations:
Elderly: Use with caution; dose should be adjusted based on renal function. Regular monitoring is required.
Children (10–12 years): Use carefully; monitor growth and puberty as long-term safety data is limited.
Renal Monitoring:
Test GFR before starting and annually if normal
2–4 times/year if near the lower normal range or in the elderly
Discontinue temporarily if renal function worsens
Contraindicated if GFR <30 mL/min
Liver Impairment: Avoid use due to the elevated risk of lactic acidosis.
Informet XR 500 should not be used in the following conditions:
Pregnancy: Uncontrolled diabetes during pregnancy increases the risk of birth defects and perinatal mortality. Insulin is preferred over metformin for managing blood glucose during pregnancy to minimize fetal risk.
Lactation: Metformin is passed into breast milk, but no harmful effects have been noted in infants. Due to limited data, breastfeeding is not recommended during treatment. The decision should weigh the benefits of breastfeeding against potential risks.
